Compounded Medication Consent

Draft for attorney review. Patients sign the final version in our patient portal before any compounded medication is prescribed.

1. What it is. Compounded medications (such as NAD+ and sermorelin) are prepared by a state-licensed pharmacy specifically for you based on your clinician's prescription.

2. Not FDA-approved. Compounded medications are not reviewed or approved by the FDA for safety, effectiveness or quality. Evidence in humans may be limited.

3. Pharmacy standards. Apex Method uses licensed, accredited 503A pharmacies that provide potency and sterility testing. Your medication is made for you only and is not for resale.

4. Risks and alternatives. Your clinician has explained known side effects, the limits of current evidence, and alternatives, including FDA-approved options and no treatment.

5. Monitoring. You agree to the follow-up visits and lab tests your clinician recommends, and to report side effects promptly.

6. Your choice. Taking a compounded medication is voluntary. You may stop at any time.
